What this policy covers
This policy covers the Carter Cares Marriage and Family Therapy PLLC website at cartercaresmarriageandfamilytherapy.com and its consultation request form. It does not cover therapy records, which are described below.
What the request form collects
The form asks for your first and last name, your email address, a phone number if you choose to add one, how you would like to meet, the type of therapy you are interested in and a short message. Please keep the message brief and leave out detailed health information. You can share more during your consultation.
How your request is delivered
When you send the form, your request is emailed to Carter Cares through a delivery service run by the practice's website provider. A limited record of each request, such as your name and email address without your message, is kept so a missed delivery can be traced. The email services that carry the notification also process its contents.
Email and web forms are not secure channels
Website forms and regular email are not encrypted from end to end. Please do not send diagnoses, medications or other sensitive details through the form or by email.
How your information is used
Your information is used only to respond to your request and set up a free consultation. It is not sold, and it is not used for advertising.
Cookies and site statistics
This site does not use advertising cookies. The hosting platform may set security cookies and record basic visit statistics, such as page views, to keep the site running.
Links to other websites
Some pages link to other sites, such as Zocdoc, Psychology Today and YouTube. Their own privacy policies apply once you leave this site.
Therapy records
If you become a client, your therapy records are kept separately from this website. They are handled under Azia Carter's obligations as a Licensed Marriage and Family Therapist in New York and under applicable law, including HIPAA where it applies.
Updating or deleting a request
To ask that a request you sent be updated or deleted, call (914) 363-0004 or email cartercarescounseling@gmail.com.
Emergencies
This website and its form are not monitored for emergencies. If you are in crisis, call or text 988, or call 911.
